Hi: When CSW throws exception, the resulting !ExceptionReport document is
not valid XML (missing xsi namespace and schema pointer).

The attached patch fixes this issue.

Note I am using svn trunk (might be a good idea to add a version of "svn-
trunk (development)" to the select box.
