Hi Francois,
for codelists I agree with Tim: we can have a fixed vocabulary whose codes
are translated by a client application. For the other elements, I don't
think that adding a 'lang' attribute makes it pass the validation (so says
the schema).
I'm currently reading the iso19115:2003 spec, maybe something will come out.

For codes like topicCategory, scopeCode etc., ISO allows for the
definition of alternative expressions in other languages, like in the
example (see attachment, taken directly from ISO19139 XSDs).
