I miss-understood your question.

I do want to start allowing graphics to be using (instead of just text
labels). This isnt in the spec, but for highway shield labeling, it
would be nice to have the labeler make the placement decision.

Others have solved this by using a custom font (ie. ESRI has some for
oil wells etc...).

Whats the result when you use a <PointSymbolizer>...<Graphic>? It
should put a graphic at the line's centroid.

As for combining text and graphics, what happens when you you do both a
<PointSymbolizer>...<Graphic> and a <TextSymbolizer>. The main problem
is that the PointSymbolizer and TextSymbolizer use different methods
for determing placement.

One solution would be to pre-generate all the shields (and use
<Filter>s), or to have a webservice that will construct a
shield-and-label for you.
