User 'format_options' to control modules of KML output

One of the ideas in the KML OWS-5 test bed is the idea of 'modules' core plus styling, extended data, ect.
There is a desire for clients to be able to request only certain modules. If servers can only return the whole thing, that's fine, but if they could understand that only 'core' was desired then it could possible make a much more terse response.
We said we'd investigate a method to do this. We can do it in a pretty custom way for now, if it's useful then others can pick it up and make it more standard.
Easiest should be to just use our 'format_options'. Can start with just extended_data=true / false to add in extended data. I think for now kml-ows5 output should have it turned on, can turn it off by adding the param. Styling is also a candidate for this, but that may be trickier to do with our current code.
