http://geoserver.org/display/GEOSDOC/Coordinate+reference+system+handling+in+Geoserver is a very valuable section of GeoServer. And a great example of why it's better to have a versioning documentation system.

It should go under 'advanced geoserver configuration'. From 'Configuring your data Coordinate Reference System' you just need the last section about 1.5.1 and onwards, don't need the previous ones. Don't need the page 'Custom projection definition in Geoserver 1.3 and 1.4', just the one that's for 1.5 and on, and don't need to put 1.5 in the name. Custom projection in postgis and hacking the epsg h2 directly should both be migrated over.
