The SLD spec does not give any limitation on the size/shape of a graphic used in a graphic fill (the <ExternalGraphic> element. As far as I can tell, the <Size> element refers to the height of the graphic, and the width is scaled accordingly.

Except that GeoServer appears to be setting both the width and the height to be the <Size> value, squishing a rectangular graphic to be square.
