This happens if the symbol is as large as the requested size (or larger and has been rescaled to fit) and the symbol is a point symbolizer.
It is due to the point geometry used to draw the symbol being placed at integer coordinates, e.g., request height 25 and the point will have a y of 12, causing the top border to end up out of the image.
