Has anybody a solution for filtering a GetLegend request so that only
legend items for features within a given bounding box are returned?
We have a dataset with tons of legend items, but usually only small
areas are investigated with a very limited number of legend items
needed...

Someone might have a better answer, but looking at the GetLegend code it does not seem possible, and would take some non-trivial modifications.
One thing we could do is optionally support the addition of a FILTER or a BBOX parameter. When specified actually do a query against the layer to figure out what features apply and then based that filter the rules applied from the SLD.
As with most feature requests, your best option is to submit it in the tracker and hopefully a developer will scratch the itch. Or if you or your organization is willing to fund such a feature request that is another option.
