1) If you download the Windows installer you should not install Tomcat or the WAR file. The Windows installer comes with a Tomcat-equivalent named Jetty and this causes collisions (on the port number), so you should uninstall Tomcat.
2) What you're seeing is Jetty , which has no content for / , hence the 404.
3) The WAR is for Tomcat and similar containers. Ignore it.
4) Default install location for the Windows installer is fine. (If you do use Tomcat / WAR then it does have to go into the webapp directory)
5) You should be able to choose the location during installation, perhaps by going through "Advanced" or customize. That would probably be a question for the postgis list.
It's a bit outdated in terms of the colors, but essentially the tutorial for adding shapefiles is http://geoserver.org/display/GEOSDOC/User+Tutorial+Shapefile (via tutorials page http://geoserver.org/display/GEOSDOC/Tutorials) . It's a bit tricky to provide the next steps after the installation, because that's where everyone start making their own choices. We're working on better documentation though.

and 1 comment: I think there is a lot of material, very (or too) detailed on (very) basic things (for example how to add a shapefile to geoserver) but then, there is a very big gap from all the bascis to actually doing some things... maybe it is just my varied background and lack of "computer" and databases knowledge, but when the tutorials or books tell me "now you are ready to go" I fell at a complete loss. I feel the jump is from "click here" to developers
