From the documentation on the web site I see that Geoserver supports GML and SHAPE output. Are any other formats supported and how easy is it to add support for additional formats?

Geoserver does support many output formats, and it is very easy to add your own. In fact we have a tutorial that describes how to do just that: http://docs.codehaus.org/display/GEOSDOC/Create+your+own+Output+Format
For WMS getMap requests, you can also retrieve: gif, png, jpeg, svg, kml/kmz, and soon: pdf
