Is it still not possible to have a perpendicular-offset option for all line based styles? Is there any estimate on how much work is required to implement this feature?

Nope, still not there, a few patches got presented but non was actually
worknig in the general case.
It would take a few days of work to implement a version that works in
general and that's fast enough
to be used for rendering
