Microsoft has released a CTP of SQL Server 2008 SP1, you might try installing this to see if it speeds up your queries. Apparently MS released SQL Server 2008 without the proper hinting logic to natively exploit spatial indexes. Our application uses spatial indexes and data types heavily. We found that SP1 has improved query performance significiantly.
