In Nviz when I need to save visualisation as maximum rosolution ppm, and I have legend added, the legend (but not the north arrow!) is tiled over the image regulary 12 times.
There are no such problems in saving as normal ppm or tif.

I tried by I cannot place a scale in grass 6.2.2 (in fact I almost have
never used that version of grass, so cannot say why) so I cannot
answer this

All render ok exept legend, anyway legend is added or not
I would suggest filing a new bug report in the bug tracker for this.
For now as a work-around you might make an image with and without the
legend, and then cup&paste the legend into the clean version in a paint
program.
