Are you perhaps trying to do this using a Mac OSX system? This has happened under OSX for a long time and no one seems to know why. The tcltk entry screen is supposed to take you to a text screen to create the new location, but it didn't. The alternative is to start grass in text mode (>grass57 -text) when you want to make a new location.
However, I just downloaded and installed Lorenzo Moretti's new binaries from 15 May 2004 and the new location button finally seems to work properly on my Mac. I will have to try this out more.
