[GRASS5] new function to catch drawn vector map available

a vector map drawn in GRASS monitor:

added D_[set,get]_dig_name()

    * D_set_dig_name(name)
    * sets the name of the dig file currently displayed
    *
    * D_get_dig_name(name)
    * returns the name of the dig file currently displayed

This is comparing to D_[set,get]_cell_name() already used by
d.what.rast.

Huidae and me currently improve d.pan to auto-redraw maps in
GRASS monitor after panning/zooming. Like that we are able to
develop an *intuitive* zoom/pan tool without annoying
d.erase; d.rast; d.vect.

I have update the GRASS 5 Programmer's Manual on that and
additionally added a new draft index in appendix referencing the
pages where a function is mentioned (thanks go to Latex :-).
Note: This index is not complete yet as the new functions description
layout is not yet applied to all chapters.
Coming soon (mmh, I hope so).
