Attached is some improved documentation for v.out.ascii. I've included
information on what the Grass 'standard' format is, largel based on the
description given in v.in.ascii. I've also included an example showing what
the 'standard' output looks like on the 'quads' vector in Spearfish.

Let me know if in the future you prefer the documentation improvements to be
attached as a diff file, or pasted within the email text itself.
